Each time you start, you get a random boss (4 bosses total), the game is an Undertale-style battle where you must avoid attacks, but instead of being turn-based, it's all real-time.
BUT
you only have 5 hearts, every boss attack will only deal 1 damage, so becaureful
Tip for the cactus chain attack: Always go down. Never up.
